Insurance & Billing

Our office provides care for many patients on various insurance plans.  We are happy to submit your claims to your insurance company on your behalf.  This is a complimentary service.  We are a provider for:   Aetna  (PPO and DMO), Cigna,  Delta Dental, Humana, Guardian  (PPO and DMO), Met Life, Safeguard (PPO and DMO) and Tricare  just to name a few.  Give our office a call if you have questions regarding your insurance.  Our staff will be happy to help determine your dental benefits as estimated by your insurance carrier.

08:10 am - 05:00 pm

Dr. Clements is ALWAYS on-call for his patients!.  Simply call our office as usual, press #1 when prompted and leave your message.  He will be paged and will get back to you as soon as possible. 

Appointments

Give our office a call and we will try to find a time that is mutually convenient.  If you can not keep your scheduled appointment, kindly give us 24 hours notice.  Appointments cancelled with less than 24 hours before the scheduled appointment time may incur charges.
